How to Register as a New Patient

Welcome to Our Practice!

We constantly are striving to make your visit more efficient. Registering as a new patient is easy. Just follow the simple steps listed below. If you have any questions just call us at (602) 258-8500. We are here to help!

 1. Please assist us in properly scheduling your appointment

Before we are able to schedule your appointment we will need the following information faxed to our office. Please contact your referring physician and ask them to fax us the information listed below. Our fax number is (602) 258-8510.

  • patient information sheet
  • copy of your insurance card (front and back)
  • authorization or referral from your insurance company
  • office visit notes
  • imaging reports
  • surgical records
  • pathology reports

2. We will call you to schedule your appointment as soon as we receive the necessary information

As soon as we receive these items from your referring physician, we will call you to schedule your first appointment. If you have not received a call from us, feel free to contact our office to check the status of scheduling your appointment. You can reach us by phone at (602) 258-8500.

Office Hours:
Monday – Thursday, 9:00am – 4:30 pm
Friday, 9:00am-12:00pm.

Lunch is taken from 12:00 to 1:30 daily at which time we are not available to reach by phone.

3. Help us serve you better by preparing these documents in advance

We strive to make your experience as smooth and convenient as possible. To help us serve you better, please fill out these new patient forms in advance of your appointment. If you are unable to do so, or would prefer to fill them out in the office, please arrive thirty minutes prior to your scheduled time. Be sure to hand carry all films or CDs of your diagnostic tests to each appointment.

4. Don’t forget your co-pay

Your co-pay is expected and collected at the time of service. Unfortunately, if you are unable to pay your co-pay at that time, your appointment will have to be rescheduled. If this is a problem for you, please discuss this in advance with Dina.

5. Cancellation Policy

We have carefully selected an appropiate time for you to meet with your doctor. If the time we have selected will not work for you, please contact our office to reschedule your appointment. As a courtesy, We require 24 hours notice for appointment cancellations. If you cancel your appointment less than 24 hours in advance, or you are a NO SHOW to your appointment, you will be charged a $40.00 fee.
